FHIR and Next-Gen Health Information Exchange (HIE)

FHIR is the simplest, most extendable, and most adopted version of healthcare interoperability standards that is also comprehensive, flexible, and intuitive. The CMS and ONC’s interoperability proposed rules mandate and progressively enforce FHIR endpoints and FHIR workflows. FHIR leads the way because it provides the healthcare industry with a breadth of standards from a data model perspective. This includes a set of APIs, a well structured way of modeling and storing the information, as well as a set of guidelines and approaches (implementation guides or IGs) that describe how to use FHIR to achieve your organization’s goals. Those goals could be how to measure and impact social determinants of health, how to build on clinical research, how to optimize and store imaging, how to securely store and transmit volumes of managed, structured data between organizations, just to name a few. 

Unlike other IT vendors who offer legacy interoperability solutions or use their own proprietary data models, outdated or legislatively unresponsive technology or cloud offerings that lock customers into expensive transactional billing models, the Smile HDF is different.

Smile and the Next-Gen HIE

The Smile HDF is open-standards compliant and is composed of three utilities, which support reducing administrative and maintenance costs, improving population health, and facilitating patient engagement. This is how: 

  • Cost Savings with the Health Data Utility (HDU) - which supports all data infrastructure (such as security, consent, search, persistence modules and the clinical data repository), data uplifting (Master Data Management or MDM, translations and transformations), and 3rd party apps and service integration capabilities (CDS hooks and interceptors). Through FHIR’s semantically permissive and structured characteristics and the HDF’s data validation capabilities, you can significantly reduce costs of maintaining feeds. Even more savings are available with guaranteed structured data capture and real time access to accurate data, due to Smile’s MDM capabilities. Over time, the HDU can substantially reduce the reported US $3B of waste due to the lack of nationwide HIE connectivity. 

  • Clinical Decision Support, Population Health Management and Analytics with the Clinical Reasoning Utility (CRU) - which supports capabilities like CQL-based clinical reasoning engine and decision support logic. This utility can improve population health and offer a suite of health data, clinical reasoning, decision support logic and analytics that pave the way for value-based care. This utility also supports digital Quality Measures (dQM), computable CPGs (clinical practice guidelines), proactive closing of care gaps, Prior Authorization and more. 

  • Patient Engagement with the Marketplace Partner Utility (MPU) - which enables SMART on FHIR apps, consent and form modules and more features that integrate with EHRs. This utility provides patients with real-time access to their data. Additionally, providers have access to real-time relevant patient information to reduce clinical burnout, enhance patient safety and care and create new efficiencies. This provides HIEs with complete, real-time access to health data that drives flexible analytics (such as care gaps, Quality Measures, population health data).
